About The Position

This role at the Family Practice Center involves a variety of clinical and administrative tasks to support patient care. The LPN/CCMA will be responsible for patient triage, managing electronic prescriptions, performing phlebotomy and EKGs, handling patient communications, scheduling appointments and procedures, and maintaining accurate patient records. The position requires strong organizational skills, critical thinking, and the ability to work both independently and as part of a team.

Requirements

  • Knowledgeable of EMR.
  • Knowledge of nursing principles and protocols and all aspects of medication names and prescribing.
  • Knowledge of policies and procedures related to infection control, environmental safety and patient confidentiality as directed by HIPAA standards.
  • Knowledge of medical chart reporting and records.
  • Skilled in customer service with a pleasant and courteous demeanor.
  • Well organized and detail oriented.
  • Able to work independently without much supervision.
  • Skills in critical thinking, communication, influence and decision making.
  • Ability to work cooperatively as a member of office staff and team.
  • High School Diploma or equivalent.
  • State Licensed LPN Nurse.
  • Current CPR Certification.

Responsibilities

  • Patient triage including weight, blood pressure, temperature readings, updating medication lists, and documenting chief complaints.
  • Manage electronic patient prescription services, including communication with pharmacies and verification of all medical prescriptions.
  • Perform phlebotomy and EKGs.
  • Answer telephone, screen calls, retrieve voicemails, and return patient calls.
  • Handle pre-certification requirements for all procedures.
  • Call patients regarding lab results.
  • Maintain daily logs.
  • Answer questions regarding patient appointments and testing.
  • Communicate with patients regarding patient information and education about procedure preps.
  • Schedule procedures, medical tests, and referral appointments, coordinating and rescheduling as necessary.
  • Verify necessary information such as demographics and insurance information for appointment scheduling.
  • Assist other staff as directed.
  • Perform other job functions as directed.
  • Attend meetings as required.
