About The Position

Home Instead is looking for an energetic, dynamic, competitive, and sales-oriented LPN to join their team as a Case Manager. This role involves guiding families through the complexities of home care, retirement, and care options. The LPN Case Manager will conduct care consultations, client/Care Pro introductions, and quality assurance visits. They will utilize a consultative sales approach to assess client needs, develop tailored service plans, and ensure high-quality care through ongoing communication and visits. This leadership position also includes teaching and training the unregulated workforce through classroom sessions and in-home visits. The role requires genuine empathy, advanced communication skills, and the ability to manage multiple client demands in a fast-paced environment. Knowledge of health care is beneficial for understanding client challenges. The position involves visiting clients and families throughout the city, with no two days being the same. Experience in community home care is specifically required, differentiating it from general practice or facility-based LPN roles.

Requirements

  • Valid LPN license and registration
  • Minimum 2 years of experience with community-based Home Care
  • Minimum 3 years of sales experience
  • Advanced verbal and written English communication skills
  • Confident communicator
  • Teaching/training experience
  • Valid minimum class-5 driver's license
  • Clear criminal background and Vulnerable Sector check

Responsibilities

  • Conduct care consultations with potential clients and family members.
  • Perform client/Care Pro introductions.
  • Conduct quality assurance visits with existing clients.
  • Use a consultative sales approach to determine individual client needs and provide solutions.
  • Create tailored service plans for clients.
  • Continuously evaluate care plans through ongoing communications and visits to ensure high-quality care, client satisfaction, and retention.
  • Identify opportunities to increase service hours.
  • Teach and train the unregulated workforce, including classroom training and in-home visits.
  • Assess client needs and develop collaborative care plans.
  • Manage multiple client demands and their changing needs.
  • Visit clients and their families throughout the city.
