About The Position

Join our team at Home Instead, the leading provider of personalized in-home care services dedicated to enhancing the lives of aging adults and their families since 1994. The LPN Case Manager will work with families who are seeking guidance regarding care options for their elderly loved ones. In this role you will use genuine empathy as well as advanced spoken and written communication skills to guide potential clients through the confusing maze of choices and care options. The LPN Case Manager role gives you the freedom to do much more, when you meet with families to assess their needs, develop collaborative care plans and continuously work with them to "navigate their care journey". This is much more fulfilling than simply leasing a room and getting a sale, this is about improving the life of our clients. To be successful in this role knowledge of home and community care is required. This role is very different than a dermatology or general practice LPN Role.

Requirements

  • Current LPN Registration
  • Experience with community-based Home Care (Care Centre/Facility experienced applicants need not apply)
  • Sales skills and experience
  • Advanced verbal and written English communication skills (NO EXCEPTIONS)
  • Teaching/training experience
  • Comfortable with a very fast paced, results oriented work environment
  • A clear criminal background check
