About The Position

Benchmark is seeking a compassionate Licensed Practical Nurse (LPN) to deliver nursing care to residents in a warm, comfortable, home-like environment. The role is part of a team focused on building a supportive community and long-lasting relationships with associates and residents. The LPN supervises Resident Care Assistants, coordinates residents’ care, and supports resident independence while maintaining safety and following state regulations.

Requirements

• Current state LPN license.
• CPR certification.
• Graduate of an approved LPN program, as required by the state.

Nice To Haves

• One to two years of experience working as a nurse is preferred.
• Previous experience working with older adults in a paid or volunteer position is highly desirable.

Responsibilities

• Accurately assess, communicate, and document residents’ status.
• Observe and report significant changes in residents’ behavior or health to the Resident Care Director, physician, family, Resident Care Assistants, and Executive Director in accordance with state regulations.
• Direct and supervise the daily work assignments of Resident Care Assistants.
• Mentor, guide, and direct Lead Resident Care Assistants in all aspects of their jobs.
• Coordinate care needs with community providers through an effective case-management process.
• Balance service demands with supporting residents’ independence.
• Recognize and help prevent elder abuse, neglect, and exploitation, and report concerns to appropriate sources in accordance with state regulations.
• Respond to emergencies and personal emergency response systems, and determine when to call for backup.
