About The Position

Community Health Network of Connecticut, Inc. (CHNCT) is seeking an LPN Care Coordinator for the Rural Health Transformation (RHT) Program. The RHT Program is a federal grant aimed at strengthening rural communities by improving healthcare access, quality, and outcomes through healthcare delivery system transformation.

Requirements

Active Licensed Practical Nurse License
Current and unrestricted motor vehicle operator's license and reliable transportation.
Excellent organizational skills
Ability to manage multiple priorities concurrently
Strong oral and written communication skills
Strong computer and keyboard skills
Team player
Ability to exercise independent and sound judgment in decision making

Nice To Haves

Managed care experience preferred
Knowledge of HUSKY Health contract requirements preferred

Responsibilities

Provide care coordination support for eligible HUSKY Health members in rural areas under the direction of the Care Management (CM) Manager of Rural Health Transformation.
Conduct targeted outreach to rural members to engage them in the CHNCT Intensive Care Management (ICM) program.
Complete designated assessment scripts, perform medication reconciliation, provide condition and medication related coaching, support care plan implementation, and assist with care coordination to facilitate access to HUSKY Health services and community resources.
Collaborate with the multidisciplinary care team to address barriers to care and support the achievement of care plan goals.
Coordinate services, address member needs, and support care plan goals in collaboration with RN care managers and team leads.
Complete risk screenings/assessment scripts to identify member needs and provide interventions.
Refer members to an RN care manager for high-risk medical, behavioral, or social issues.
Respond to telephonic inquiries from members.
Coach and assist members in achieving successful self-management skills.
Provide appropriate written/digital educational resources.
Participate in interdisciplinary case rounds/collaborative meetings with the Rural Health Transformation team.
Complete documentation in the electronic care management documentation system.
Perform other duties as assigned.
Conduct telephonic/videoconference member outreach, structured assessments, and coaching.
Participate in virtual member learning groups and organized community activities upon request.
Maintain knowledge of covered benefits for CT Medicaid and HUSKY Health programs.
