About The Position

Community Health Network of Connecticut, Inc. (CHNCT) is currently seeking two (2) Care Support Coordinators for the Rural Health Transformation (RHT) Program. The Rural Health Transformation (RHT) Program is a federal grant that empowers states to strengthen rural communities across America by improving healthcare access, quality, and outcomes by transforming the healthcare delivery ecosystem.

Requirements

  • High School diploma or GED required
  • 1 to 3 years’ healthcare related experience
  • Member outreach experience
  • Excellent organizational skills
  • Ability to manage multiple priorities concurrently
  • Strong computer (use of Microsoft office products, Word, Excel etc.) and keyboard skills
  • Excellent, concise, accurate documentation skills
  • Knowledge of CT Medicaid managed care system

Responsibilities

  • Supporting the clinical Care Management (CM) staff in coordinating the outreach and care of CHNCT's members.
  • Researching and assigning referrals to the CM clinical teams as assigned, performing outreach to members to complete non-clinical screening assessments, and carrying out care coordination tasks.
  • Serving as an administrator for the virtual training program, including user and module set-up.
  • Using referral assignment criteria developed by clinical staff, researching referrals as indicated, and assigning referrals from multiple sources, as assigned.
  • Receiving referrals/assignments, outreaching to members, completing non-clinical screenings and identifying appropriate members for the CM Programs per policy.
  • Coaching members on use of HUSKY Health program, services, resources and ASO services.
  • Emailing/mailing educational materials and resources to members as needed.
  • Demonstrating timely and accurate documentation in the care management documentation platform.
  • Contacting Case Managers for clinical support, guidance and direction, as needed.
  • Assisting members in finding a primary care provider (PCP) and scheduling appointments.
  • Outreaching to PCPs to verify attribution and assist with scheduling member appointments.
  • Faxes provider letters created by clinical staff to attributed PCP.
  • Distributing faxes to appropriate staff.
  • Providing information about transportation benefit and services and assisting as needed.
  • Completing documentation in the electronic care management documentation system.
  • Contacting appropriate state agencies to inform waiver personnel of member hospital admission, as assigned.
  • Providing clerical and administrative support to CM teams as needed.
  • Serving as an administrator for the virtual training program, including user and module set up, and associated enrollment tasks.
  • Participating in meetings as appropriate with other members of the Rural Health Transformation team.
  • Works collaboratively with internal staff; is able to establish and maintain good working relationships with members, providers, colleagues.
  • Supports and trains peers with assignments and tasks as needed.
  • Cross trains to non-clinical tasks performed by all CM teams to provide support when indicated. Demonstrates adherence to their scope of service and communicates appropriately to care managers for guidance and support.
  • Performs other duties as assigned.
