About The Position

The nurse coordinates and leads the patient care team to develop, implement, and evaluate the plan of care in accordance with the Illinois Nurse Practice Act. The role works Monday through Friday, 7:00 a.m. to 3:30 p.m. Pay is based on experience, with an estimated compensation range of $23.87 to $37.00 per hour.

Requirements

High school education.
BLS or CPR certification within 90 days of hire.
Graduation from an accredited LPN program.
Illinois Licensed Practical Nurse (LPN) license through the Illinois Department of Financial and Professional Regulation.

Responsibilities

Administers medications and treatments according to protocol.
Initiates a plan of care at admission, focusing on the patient’s identified physiological, psychological, and environmental factors.
Identifies patient problems on integrated flowsheets and documents progress against the plan of care.
Coordinates discharge planning with all team members.
Handles emergency situations promptly, precisely, and professionally.
Documents accurate and ongoing assessments of patient status every two hours or as needed.
Documents patient teaching and education about the plan of care.
Initiates appropriate pathways for inpatient physician orders.
Monitors caregiver productivity and provides or recommends education when needed.
Notifies team members when an assessment is required by physician order.
Performs patient assessments each shift and documents them at the time of assessment.
Provides and coordinates patient and family education and documents it appropriately.
