About The Position

The LPN Admissions Representative is a full-time position responsible for conducting preadmission discussions, documenting patient information in the EMR, triaging nursing-related calls, and supporting Admissions Nurses. This role ensures patients with the greatest needs are prioritized and that all arrangements for post-discharge visits are prepared.

Requirements

  • Graduate of an approved school of practical nursing
  • Maintains current Pennsylvania nursing license.
  • One year of nursing experience as a licensed practical nurse within last four years required

Responsibilities

  • Conducts remote preadmission discussions with patients and families to capture and document patient story and schedules the visit based on level of complexity and urgency.
  • Enters nursing related information into the electronic medical record (EMR), including allergies and pertinent diagnoses.
  • Triages phone calls coming into Intake & Admissions Office to answer nursing related questions and prioritize admission visits to ensure patients with the greatest needs are escalated.
  • Makes follow up calls and remote chart reviews to hospitalized patients awaiting discharge to ensure all arrangements are prepared for post-discharge visit including medications, durable medical equipment (DME), transportation, and any other needs.
  • Supports Admissions Nurses by ordering equipment, supplies, and medications.
  • Available to cover admission and follow up visits based on staffing and referral volumes.
