About The Position

The Center for Perioperative Medicine is seeking a motivated LPN to support preoperative patient care coordination and workflow management. This role partners with surgeons, anesthesia, APPs, specialty teams, and external facilities to help support patients through the surgical preparation process. Responsibilities include coordinating testing and follow-up appointments, communicating with interdisciplinary teams, reinforcing patient education and provider instructions, addressing barriers to care through escalation to the appropriate RN or provider, and supporting cross coverage of the anticoagulation management program. Candidates must have strong communication, organizational, and critical thinking skills, attention to detail, and a collaborative, patient-centered approach. Responsibilities

  • Remains accountable for provision of direct care to assigned patients in an outpatient setting.
  • Contributes to unit activities and supports enhancement of the care delivery system.
  • Works as a team member and assists other staff members with implementing patient care.
  • Assumes responsibility of care for assigned patients under the direction of the co-assigned RN or MD.
  • Participates in the plan of care, implements appropriate nursing interventions, and contributes to patient/family education through collection of data, interdisciplinary team collaboration, documentation and excellent communication.
  • Coordinates testing and follow-up appointments.
  • Communicates with interdisciplinary teams.
  • Reinforces patient education and provider instructions.
  • Addresses barriers to care through escalation to the appropriate RN or provider.
  • Supports cross coverage of the anticoagulation management program.
