About The Position
We are seeking dedicated Licensed Practical Nurses (LPNs) to join our team, providing compassionate care for pediatric and adult clients in a home care setting across Long Island.
Requirements
- Current New York State LPN license
- 6+ months of experience
- BLS certification (American Heart Association)
- Physical exam within the past year
- QuantiFERON within the past year
- MMR Titers within the past ten years
Responsibilities
- Provide direct patient care, including administering medications and treatments
- Monitor and document patient health status and communicate changes promptly
- Support clients with activities of daily living while maintaining their comfort and dignity
- Collaborate with families and the care team to implement personalized care plans
