About The Position
Lead patient care coordination and behavioral health operations as an LPC-S/Registered Nurse, while making a meaningful impact in a collaborative hospital setting near Gallup, NM. This is a full-time, permanent position with a Monday-Friday schedule. The LPC-S/Registered Nurse will manage patient referrals and transfers to outside hospitals, ensuring continuity of care and timely placement. They will also provide clinical leadership while supporting behavioral health and interdisciplinary care teams, and coordinate with physicians, case managers, and healthcare professionals to optimize patient outcomes. Requirements
- Active Licensed Professional Counselor Supervisor (LPC-S) license or Registered Nurse (RN) license required
- Excellent communication, critical thinking, and interdisciplinary collaboration skills
Nice To Haves
- Bachelor of Science in Nursing (BSN) preferred
- Previous leadership, care coordination, or supervisory experience preferred
