About The Position

The Social Worker position assists and supports patients and their families, provides psycho-social assessments to identity areas of need, to then provides assistance as able to those areas of need, and offers and secures different resources as needed and able; all while serving as the patients' advocate and honoring their choices and rights.

Requirements

  • Bachelor of Social Work degree required.
  • Must hold and maintain Wisconsin Social Worker licensure.
  • 1+ years of related work experience required.
  • Exceptional accuracy and attention to detail required.
  • Intermediate to proficiency with computers is required.
  • Knowledge of Medicare and Medicaid systems.
  • Ability to maintain confidentiality.
  • Excellent oral and written communication skills.
  • Self-starter with excellent compassion, interpersonal communication, and problem-solving skills.
  • Ability to have continuous oral and written communication with co-workers and supervisors is essential.

  • Specialty certifications preferred.

Responsibilities

  • Assist patients and families with discharge planning primarily in acute care and other departments as needed.
  • Work with patients and their families to provide psycho-social assessments to identify areas of need. Then assist/offer/secure options, services, resources etc. to help with those areas of need.
  • Serve as patients' advocate and advocating for/honoring patient's choices and rights.
  • Assist residents and families in understanding insurance coverage and assisting in medical assistance applications as well as collaboration with community resources.
  • Assist residents and their loved ones with medical assistance reviews to keep their coverage.
  • Receive and review grievances reported by residents, loved ones, and staff.
  • Consult and work with the IDT and other staff on patient/family situations, their needs, patient care, and discharge plans.
  • Complete needed, required, and professional documentation and paperwork for documentation of patients' charts and other jobs tasks and requirements.
  • Complete admission paperwork and discuss policies and procedures with residents and their loved ones upon admission.
  • Organize and schedule discharge. Direct discharge planning meetings. Assist residents in making connections with community resources to ensure a safe and successful discharge.
  • Perform other duties as requested.
