Long Term and Post-Acute Care APP
About The Position
MedCap Health is seeking a part-time LongTerm and Post-Acute Care APP to help reshape the future of post-acute care. This role offers flexibility, autonomy, and a true sense of purpose. As a key member of a multidisciplinary team, you will play a central role in patient care, managing patients with acute and chronic diseases and collaborating with the attending physician. Your expertise will be crucial in managing patients with debilitating chronic diseases and improving their quality of life. You will also collaborate with the Director of Nursing, Social Workers, and other team members to ensure a cohesive approach to care.
Requirements
- Board-certified geriatric, family, or adult Nurse Practitioner (ANCC, ACHPN, or AANP)
- 1+ years of experience in post-acute/SNF/LTC
- Prior post-acute, long-term, and/or skilled nursing facility experience
Responsibilities
- Managing patients with acute and chronic diseases
- Collaborating directly with the attending physician
- Managing patients with debilitating chronic diseases
- Communicating and working seamlessly with the Director of Nursing, Social Workers, and other team members
