About The Position
MedCap Health is seeking a part-time/full-time/flexible Long Term and Post-Acute Care APP to help reshape the future of post-acute care. This role offers flexibility, autonomy, and a true sense of purpose. As a key member of a multidisciplinary team, you will play a central role in patient care, managing patients with acute and chronic diseases and collaborating directly with the attending physician. Your expertise is crucial in effectively managing patients with debilitating chronic diseases, improving their quality of life. You will communicate and work seamlessly with the Director of Nursing, Social Workers, and other team members to ensure a cohesive and comprehensive approach to care. This dynamic role keeps you engaged and connected as you make a tangible difference in the lives of those you serve.
Requirements
- Board-certified geriatric, family, or adult Nurse Practitioner (ANCC, ACHPN, or AANP)
- 1+ years of experience in post-acute/SNF/LTC
- Prior post-acute, long-term, and/or skilled nursing facility experience
Responsibilities
- Manage patients with acute and chronic diseases.
- Collaborate directly with the attending physician to provide the highest quality of care.
- Effectively manage patients with debilitating chronic diseases, improving their quality of life.
- Communicate and work seamlessly with the Director of Nursing, Social Workers, and other team members to ensure a cohesive and comprehensive approach to care.
