Long Term and Post-Acute Care APP
About The Position
MedCap Health is seeking a full-time Long Term and Post-Acute Care APP to provide care in post-acute settings. The APP manages patients with acute and chronic diseases, collaborates with the attending physician, and works with a multidisciplinary team to support high-quality, comprehensive care. The role focuses in part on patients with debilitating chronic diseases and improving their quality of life. The schedule is Monday through Friday, during day shifts, with no nights or weekends required. Requirements
- Board-certified geriatric, family, or adult Nurse Practitioner (ANCC, ACHPN, or AANP).
- At least 1 year of experience in post-acute, skilled nursing facility, or long-term care.
- Prior post-acute, long-term care, and/or skilled nursing facility experience.
Responsibilities
- Manage patients with acute and chronic diseases.
- Collaborate directly with the attending physician to provide high-quality care.
- Work with the Director of Nursing, social workers, and other team members to coordinate comprehensive patient care.
- Help manage debilitating chronic diseases and improve patients’ quality of life.
