Long Term and Post-Acute Care APP
About The Position
MedCap Health is seeking a full-time Long Term and Post-Acute Care APP to provide care in post-acute settings. The role offers flexibility, autonomy, administrative support, and a sense of purpose. The APP manages patients with acute and chronic diseases and works directly with the attending physician and a multidisciplinary team to deliver high-quality, coordinated care. The position focuses in part on patients with debilitating chronic diseases and supporting their quality of life. The schedule is Monday through Friday, day shifts, with no overnight or weekend work required. MedCap Health is a physician-led organization providing services across specialties including pulmonology, nephrology, psychiatry, and internal medicine. It partners with more than 200 skilled nursing facilities across 11 states.
Requirements
- Board-certified geriatric, family, or adult Nurse Practitioner (ANCC, ACHPN, or AANP).
- At least 1 year of experience in post-acute, skilled nursing facility (SNF), or long-term care (LTC).
- Prior experience in post-acute care, long-term care, and/or a skilled nursing facility.
Responsibilities
- Manage patients with acute and chronic diseases.
- Collaborate directly with the attending physician to provide high-quality care.
- Communicate and work with the Director of Nursing, social workers, and other team members to coordinate comprehensive patient care.
- Help manage patients with debilitating chronic diseases and improve their quality of life.
