Patient Engagement Advisors (PEA) is seeking a full-time Logistics Transition Specialist to work onsite at AdventHealth Tampa and in the local area while performing Logistics Courier duties. The Logistics Transition Specialist plays a vital role in obtaining the goals of the vision/mission of PEA. To accomplish these goals, the Transition Specialist must exhibit empathy, innovation, collaboration, teamwork, passion, dedication to personal development, and a strong commitment to “know the patient”. The Logistics Transition Specialist is responsible for encouraging patients to play an active role in their patient care experience. These efforts occur during and after the patients’ hospital stay. The Transition Specialist will ensure the patient is provided services, educated on their discharge requirements (medications, patient follow-up care, nutrition, transportation, assistance programs, etc.), and equipped with the resources necessary to be successful in his/her next stage of care. The Logistics Transition Specialist is responsible for delivering medications, nutrition, product kits and other patient care related items as needed to home or another hospital locations for hand off. Position responsibilities of the Logistics Transition Specialist require access to Protected Health Information (PHI). Patient Engagement Advisors’ team members are expected to remain compliant with all confidentiality and security policies/procedures related to use, access, and disclosure of PHI.
