About The Position

Navitas Healthcare, LLC is seeking an experienced LMSW or RN Case Manager to support acute care operations. This role is responsible for discharge planning, care coordination, and utilization management. The Case Manager will work closely with interdisciplinary teams to improve patient outcomes, reduce length of stay, and ensure safe transitions of care.

Requirements

• 1+ year acute care experience (required)
• Experience in discharge planning and care coordination
• Strong clinical judgment and communication skills
• Ability to work in fast-paced hospital settings
• MSW or RN degree required
• Active LMSW or RN license (WA or pending accepted)
• BLS certification required

Nice To Haves

• Experience with Med-Surg, ED, or LLOS preferred
• EPIC EMR experience preferred

Responsibilities

• Coordinate discharge planning and patient transitions
• Collaborate with physicians and care teams
• Assess patients and develop care plans (Med-Surg, ED, LLOS)
• Monitor progress and adjust care plans as needed
• Ensure proper utilization of healthcare services
• Maintain accurate documentation in EPIC EMR
• Support compliance and quality improvement initiatives
• Float across nearby units/campuses as needed
