About The Position

Navitas Healthcare, LLC is seeking a dedicated LMSW or RN Case Manager to coordinate patient care, facilitate discharge planning, and ensure appropriate utilization of healthcare services in an acute care setting. This role partners closely with interdisciplinary teams to improve patient outcomes, reduce length of stay, and ensure seamless transitions of care.

Requirements

Minimum 1 year of acute care experience
Experience in discharge planning and care coordination
Strong knowledge of case management workflows
Ability to thrive in a fast-paced hospital environment
Excellent communication and teamwork skills
Master’s Degree in Social Work (MSW) or Nursing Degree (RN)
Active LMSW or RN License (WA or pending accepted)
BLS Certification (Required)

Nice To Haves

Experience with Med-Surg, Emergency Department (ED), and LLOS cases
EPIC EMR experience
Diverse case management background

Responsibilities

Lead discharge planning and coordinate patient care transitions
Collaborate with physicians, nurses, and interdisciplinary teams
Ensure effective utilization of healthcare resources
Connect patients and families with appropriate community resources
Assess patient needs and develop individualized care plans
Monitor progress and adjust care plans as needed
Support complex cases including Med-Surg, ED, and long length of stay (LLOS) patients
Maintain accurate and timely documentation in EMR systems (EPIC preferred)
Ensure compliance with hospital policies and regulatory standards
Participate in audits and quality improvement initiatives
Float across multiple hospital units as needed
Collaborate across departments to support overall case management operations
