Join our team as a Living Well Coordinator and lead innovative, engaging programs that enrich the lives of residents living with memory loss. The Living Well Coordinator (LWC) is the process owner of the American House Living Well Memory Care Program. This role is responsible for developing, implementing, and evaluating a person-centered activities program that promotes physical, cognitive, social, spiritual, creative, sensory, and life skills wellness. The LWC builds meaningful relationships with residents, families, and team members while creating a vibrant and engaging Memory Care environment.
Requirements
- Related degree or experience in Memory Care and/or Dementia Care.
- Minimum of 2 years of dementia and memory care experience preferred.
- Experience implementing programs for a Memory Care population.
- Ability to lead activities for residents with varying cognitive and functional abilities.
- Strong computer, organizational, and time-management skills.
- Excellent communication, interpersonal, problem-solving, and teamwork skills.
- Flexible schedule and a passion for supporting residents, families, and team members.
- Experience motivating others through fun, engaging, and meaningful activities.
Responsibilities
- Develop, implement, and oversee a diverse, therapeutic 7-day-a-week activity calendar.
- Lead all aspects of the Living Well Program and continuously enhance programming using the Memory Care Playbook and performance goals.
- Coach and support team members on the importance of resident engagement and how to lead activities when the LWC is unavailable.
- Conduct orientation for new team members on Living Well policies, procedures, and programming expectations.
- Provide ongoing education and support for Memory Care team members and resident families.
- Welcome and orient new residents and their families to the community.
- Foster a culture of compassion, engagement, and person-centered care within the Memory Care neighborhood.
- Perform other duties as assigned consistent with the position.
