About The Position

The Life Skills Instructor is responsible for serving individuals in a supportive and guiding role to assist them in mastering independent living skills. This role involves implementing all Agency policies and procedures pertaining to the safety and welfare of the individuals, and acting as a role model by promoting fun, safe, and healthy activities. The instructor will monitor consumer behavior, document activities and incidents, maintain the safety and cleanliness of the home, and ensure compliance with all relevant regulations and policies. This is a field position requiring travel.

Requirements

  • Minimum possession of a High School Diploma or a G.E.D.
  • Minimum two years of experience working with individuals with developmental disabilities or that are dually diagnosed (experience may be substituted with education on a year-for-year basis).
  • Valid driver's license.
  • Ability to be insured by the insurance company throughout employment.
  • Excellent interpersonal skills.
  • Ability to obtain and maintain certifications in CPR, First Aid, PMT, and DDS Medication Administration.
  • Ability to lift up to 75 pounds, transfer, and physically intervene with individuals as required.
  • Proof of car insurance.

Responsibilities

  • Monitor consumer behavior and implement behavior management programs.
  • Document all shift activities, incidents, and consumer progress in Therap and on appropriate forms.
  • Provide programmatic and statistical reports as required.
  • Maintain the safety, appearance, and cleanliness of the home, conducting inspections and reporting maintenance needs.
  • Maintain a professional attitude with colleagues, management, and external agencies.
  • Ensure knowledge of and adherence to all Agency policies and procedures, including medication administration, key control, IP implementation, emergency procedures, licensing regulations, and DDS contracts.
  • Alert appropriate personnel immediately regarding problems or unusual occurrences and complete incident reports accurately and in a timely manner.
  • Implement and adhere to all policies, procedures, and commitments required by the Connecticut Department of Developmental Services, ASI, and other state and federal agencies.
  • Report at staff meetings on assigned duties.
  • Act as an advocate and role model for consumers, treating individuals with respect and dignity.
  • Assist consumers with personal hygiene needs.
  • Provide counseling or reinforcement to assigned individuals regarding progress, problems, and interests.
  • Supervise and safely transport residents to community activities, day programs, jobs, errands, and social events.
  • Follow emergency procedures for fire, hurricanes, power outages, accidents, or other serious incidents affecting individuals, and notify supervisors immediately.
  • Assist guardians, parents, and others in understanding and implementing normalization/person-centered-planning principles.
  • Assist individuals in mastering independent living skills such as personal management, household management, nutritional awareness, academic skills, money management, human sexuality, safety and health, community participation, social skills, and time management.
  • Assume responsibility for an assigned caseload, including goal work, program planning, and daily/weekly/monthly paperwork.
  • Establish and adhere to a work schedule within approved guidelines.
  • Report and respond to any violation of individuals' rights directly to the Executive Director or designee and participate in investigations as directed.
  • Attend in-service training, seminars, and other professional development opportunities.
  • Provide training for Agency personnel as required.
  • Actively participate in staff meetings and trainings.
  • Distribute all medications according to schedules and guidelines.
  • Assist in shopping, meal preparation, set-up, and clean-up when on duty.
  • Follow and document Behavior Support Plans.
  • Perform other duties as assigned by the Individualized Supports Manager or upper management.
