About The Position

The West Haven Community House is seeking dedicated Life Skills Coaches to work with individuals in their Residential Program and DDS funded Day program. This role involves providing direct care and support, working independently to meet individual needs, and collaborating as part of a team. The Life Skills Coach is responsible for assisting individuals with activities of daily living (ADL), socialization, behavior management, personal safety, and goal achievement, while maintaining a clean, safe, and healthy environment. The position requires staying on shift until relieved and completing all responsibilities.

Requirements

  • High School Diploma
  • 2 years of related experience with adults with intellectual disabilities preferred
  • Must be 21 years old or older
  • Certification in CPR/First Aid required prior to hire
  • Valid CT Driver’s license, clean driving record and ability to obtain and maintain approval from leased vehicle company
  • Must obtain and maintain all certifications such as Physical Management Techniques (PMT) DDS Medication Certification, etc. as required by DDS standards

Responsibilities

  • Ensures a safe and accessible environment for all individuals.
  • Implements all safety protocols and ensures the wellbeing of all individuals while adhering to agency and state guidelines, policies, and procedures.
  • Provides incidental teaching to model appropriate behavior and activities to enhance individual independence.
  • Provides transportation for individuals as needed, including driving agency or personal vehicles.
  • Responsible for the personal hygiene of assigned individuals, including bathing, showering, dressing, and cleaning.
  • Implements individuals' eating/dietary and food consistency guidelines.
  • Ensures appropriate accountability supervision for individuals to maintain their health and safety.
  • Implements strategies in Behavioral Support Plans and/or Behavioral Guidelines for individuals.
  • Acts as an advocate for individuals, helping them make choices and ensuring their wishes are considered.
  • Maintains accurate records of required training and certifications, ensuring timely recertifications.
  • Completes accurate programming and documentation, including Electronic Visit Verification System, Therap documentation, BER, GER, T-logs, program documentation, data collection, financial records, and time tracking.
  • Establishes and maintains professional relationships with community contacts to foster inclusion and facilitates community integration for individuals.
  • Logs in hours worked using the ADP biometric time clock system.
  • Performs all assignments as outlined in the Life Skills Coach handbook and assigned by supervisor.
  • Other duties as assigned.
