About The Position

This position involves teaching clients essential life skills to promote independence within the community. Responsibilities include assisting clients with accessing community resources, medication adherence, interpersonal skills, behavioral support, crisis coping, emotional regulation, transportation access, financial management, and independent shopping. The role also requires completing documentation in the EMR within 24 hours of service delivery, conducting activities that support maximum independence, enhancing community living through skills training and supervision as per treatment plans, and maintaining all required trainings and supervision. Participation in therapeutic rehabilitation and 1915i Rise services is also part of the role.

Requirements

  • High School diploma or GED
  • One year of full-time experience working with individuals who receive services for treatment of a behavioral health disorder.
  • Valid driver's license with 2 years of driving experience.
  • Conscientious, Dependable, Good computer skills, Works well with other staff, Good communication skills.
  • Ability to work independently.
  • Good organizational and time management skills.

Responsibilities

  • Teaching clients skills such as utilizing/accessing community resources, medication adherence, interpersonal skills, behavioral support skills, crisis coping skills, emotional regulation skills, accessing transportation, budgeting/managing finances, and independent shopping.
  • Complete documentation in EMR within 24 hours of service delivery.
  • Conduct activities necessary to allow individuals to live with maximum independence in the community.
  • Enhance community living through utilizing skills training, cueing or supervision as identified in recipients treatment plan.
  • Maintain all trainings and required supervision.
  • Participate in therapeutic rehabilitation and 1915i Rise services as needed.
