We are committed to providing a great work environment for our Team Members. We are seeking a dedicated Life Services Coordinator - CMA/CNA/Non-Certified to join our Maggie's Place Memory Care team. The Life Services Coordinator is responsible for cultivating an environment that maximizes each resident’s independence by delivering assistance with personal care, medications, social and recreational activities, meals, and other tasks as needed to facilitate the continued well-being of tenants.

Requirements

  • Must have 1-2 years’ previous experience working with individuals with dementia, preferably in a memory care setting.
  • Must be at least 18 years of age, with a high school diploma or equivalent GED.
  • Medication Manager OR Certified Medication Aide recommended.

Responsibilities

  • Provides services to tenants as assigned and as indicated on Resident Service Plans which may include grooming and dressing guidance, assistance with bathing, and medication reminders/assistance.
  • Reports any significant tenant changes to the nurse.
  • Monitors for changes in tenant needs, preferences, and/or health status, and reports/documents any changes according to established procedures.
  • Reviews the Staff 24-Hour Communication log and corresponding Tenant Service Notes on a daily basis for information about changes in tenant service needs and/or health conditions.
  • Prepares and serves meals/snacks, monitors food expiration dates and maintains kitchen cleanliness.
  • Completes various housekeeping duties including; dusting, vacuuming, cleaning bathrooms and common spaces, maintaining kitchen cleanliness, make and change bed linens, laundry services.
  • Maintains activity calendar and engages tenants in activities such as spiritual, educational, games, music, and physical activities.
  • Provides companionship.
  • Works effectively with minimal supervision and in a team setting.
  • Follows safety policies in performing care, assuring resident safety.
  • Follows and upholds policies and procedures.
  • Attends and participates in facility meetings and in-service training programs as assigned.
  • Follows infection control guidelines.
  • Observes residents’ rights including, but not limited to, privacy, dignity, etc.
