About The Position

This position is responsible for providing guidance, education and support to young people in determining the course of their life course and to plan goals regarding education, vocation, living situation as well as other life skills.

Requirements

  • 3 - 5 years of experience working with young adults with mental illness and/or substance abuse issues
  • Experience in writing incident reports is a must

Nice To Haves

  • Bachelor’s Degree in related fields such as social work, psychology, education counseling is preferred

Responsibilities

  • Provide positive role-modeling to consumers, including promoting self-esteem, healthy boundaries, good hygiene, socially acceptable behaviors, and thoughtful decision-making.
  • Teach and reinforce professionalism in work tasks and employment.
  • Assess consumers' history of trauma, strengths, and needs.
  • Develop and monitor Individualized Service Plans.
  • Create plans and track strengths and needs.
  • Coordinate services and collaboration with various agencies and programs.
  • Make necessary referrals to meet consumer's health, well-being, education, mental health, and employment needs.
  • Facilitate and/or participate in Team Meetings for all consumers.
  • Collaborate and communicate with other programs within the CHRIS 180 continuum.
  • Collaborate and consult with other providers and entities.
  • Monitor the provision of services provided.
  • Meet with consumers weekly.
  • Evaluate individual progress and communicate with consumers and other stakeholders.
  • Transport residents to and from service-related activities.
  • Facilitate life skill activities such as conflict resolution, anger management, medication management, and tutoring.
  • Provide emotional support in managing daily life.
  • Communicate with resident advisors, program management, and/or therapists regarding services and needs of residents.
  • Aid in supporting clients while in The Spot (laundry, hot meals, computer lab).
  • Audit charts as assigned.
  • Maintain and record receipts.
  • Provide client updates in Case Notes.
  • Complete Incident Reports as needed prior to shift ending.
  • Complete Monthly Summary Reports and Purposeful Visits in a timely fashion.
  • Record in myEvolv portal within 48 hours of contact.
  • Email monthly summaries to CSM and ILP coordinators by the 10th of the month following the reporting period.
  • Ensure medical/dental/clinical appointments are scheduled on the calendar.
  • Maintain confidentiality of consumer information.
  • Maintain appropriate professional boundaries.
  • Follow the CHRIS 180 Code of Ethics.
  • Follow mandated reporting requirements of GA Code.
  • Support and maintain an environment of emotional and physical safety for consumers, staff, and other stakeholders.
  • Demonstrate customer service by promoting a positive image of CHRIS 180 programs.
  • Work as a contributing and collaborative team member.
  • Respond to consumer and peer needs in a timely fashion.
  • Answer telephone calls and emails accordingly.
  • Respond to requests for information from stakeholders in a timely fashion.
  • Attend and participate in staff meetings as scheduled.
  • Attend and participate in required training as scheduled.
  • Maintain required certifications.
  • Review, understand, and be responsible for program policies.
  • Provide documents in a timely fashion to Human Resources.
  • Report changes in status to HR as required by policy.
