Requirements

• High School diploma or equivalent with four years working in the human services field, and one year working with individuals with disabilities OR Bachelor’s degree in Human Services and one year experience working with individuals with disabilities.
• Must be able to work a flexible schedule, be available during evenings & weekend hours.
• Demonstrated competencies in active listening, teaching, task analysis, behavior management, curriculum development and person-centered planning.
• Ability to make good decisions in a timely manner based on analysis, wisdom, experience and judgment.
• Ability to think on your feet.
• Ability to lift 50 lbs.
• Professional written and verbal communication skills.
• Ability to work with outside organizations.
• Proficiency in or knowledge of using computer software and email applications.

Responsibilities

• Provide direct training services and professional supervision over the daily programs provided by the direct service personnel.
• Provide person-centered case management services for the individuals we serve.
• Train and supervise staff to support individuals acquiring skills in areas of their person-centered plans including but not limited to independent living skills, skills to support successful community participation, communication skills, self-advocacy, personal and social skills.
• Provide guidance and feedback to support staff growth so they are empowered.
• Conduct team meetings, prepare reports for and participate in planning meetings for individuals supported.
• Develop relationships with Community members for potential job opportunities.
• Develop person-centered goal plans and instructional plans for individuals.
