About The Position

Moorings Park is seeking a dedicated and talented Life Care Manager to join our team. The Life Care Manager provides home and environmental support services by performing comprehensive assessments, developing and implementing tailored care plans, coordinating the individual plan, and evaluating services. The Life Care Manager modifies the care plan when necessary, based on health, family dynamics, public and private resources, and funding sources while advocating for the individual.

Requirements

  • Excellent oral and written communication skills in the English language required.
  • During emergencies or natural disasters, may be required to remain on duty for the duration of the event.
  • A current Florida driver’s license, clean driving record, proof of insurance and reliable transportation are required
  • Associate degree in a field related to care management required (counseling, gerontology, human services, nursing, psychology, rehabilitation, social work, occupational, physical, respiratory or speech language therapy degrees would relate)

Nice To Haves

  • Bachelor’s or master’s degree in a field related to care management preferred
  • Advanced certifications preferred (Certified Care Manager, Certified Geriatric Care Manager or Case Manager and/or Certification in Gerontology)
  • Previous geriatric, care management, and home health experience preferred, or experience in a community-based healthcare facility with geriatric clients

Responsibilities

  • Performs initial evaluation and continual re-evaluation of the individual’s medical/health maintenance, psychosocial/behavioral/memory status, functional abilities, safety/home environment, financial/legal situation, community involvement/relationships, and ongoing care management needs
  • Develops and implements a care plan tailored to the client’s needs
  • Refers client/family to resources specific to the client’s needs and issues, such as home health, equipment, mental health and psychosocial resources, transportation, medical and housing resources, maintenance of a safe physical environment, and educational materials
  • Acts as a liaison and client advocate for the coordination of those involved in the client’s care, including healthcare providers, trust departments, guardians, facility staff, and other services and providers
  • Transports and/ or accompany client to medical and business appointments
  • Provides written updates and ongoing education to client and family members
  • Completes timely documentation of reports, letters, and other materials, including initial assessments, care plans, care notes, updates, and correspondence
