Requirements
- Graduate of an accredited Associate Degree Nursing program
- Current state LVN/LPN license in good standing
- Minimum of one (1) year of clinical nursing experience
- Ability to lift, push, pull, and support up to 50 pounds as needed
- Valid driver's license, reliable transportation, and current auto insurance
- Willingness to travel locally (approximately 50%)
Nice To Haves
- Home Health or Hospice experience preferred
- Strong clinical assessment and communication skills
Responsibilities
- Providing skilled nursing care under the direction of an RN and physician
- Following the established Plan of Care and physician orders during each patient visit
- Monitoring patient conditions and reporting significant changes to the RN Case Manager and care team
- Educating patients and caregivers on disease management, medications, and self-care techniques
- Supporting patient safety through individualized care planning and risk prevention strategies
- Documenting patient assessments, interventions, and medication updates accurately and timely
- Collaborating closely with physicians, RNs, and interdisciplinary team members to ensure quality outcomes
- Identifying opportunities to improve patient care and recommending updates to the Plan of Care
- Helping reduce avoidable hospitalizations and emergency room visits through proactive patient management
- Delivering compassionate, respectful, and professional care to patients and families
