About The Position

The Apprentice Licensed Practical Nurse (LPN) at St. Mary Rehabilitation Hospital is joining a vital mission dedicated to making communities healthier. This role is within a 50-bed inpatient rehabilitation hospital, jointly operated by Trinity Health Mid-Atlantic and Lifepoint Rehabilitation. The hospital specializes in helping patients recover from stroke, brain injury, neurological disorders, orthopedic trauma, and other complex conditions. Requirements

  • Current Pennsylvania License as a Practical/Vocational Nurse
  • Graduated from an accredited licensed practical/vocational nurse program
  • CPR/BCLS certification required

Nice To Haves

  • ACLS preferred
  • IV Certification required or obtained within 6 months (highly preferred)
  • Minimum six months’ Medical/Surgical experience in an acute care setting preferred
  • Excellent oral and written communication and interpersonal skills

Responsibilities

  • Administer medications safely, accurately, and based on patient age and clinical condition.
  • Minimize potential for medication errors by following proper medication administration procedures.
  • Assist the RN with initial assessments and patient reassessments per policy.
  • Identify and implement appropriate rehabilitation nursing interventions.
  • Perform timely and accurate Quality Improvement (QI) assessments under RN supervision.
  • Deliver care based on patient response to treatment and update the interdisciplinary plan accordingly.
  • Assess and reassess pain; use appropriate pain management techniques and educate patients and families.
  • Perform all patient care tasks in a manner that promotes patient safety and reduces medical errors.
  • Develop and implement individualized teaching plans based on patient and family learning needs.
  • Perform other duties as assigned.
