Requirements
- Active LPN license in good standing
- Graduate of approved nursing program
- 1+ year experience in senior care or clinical setting
- Strong assessment, documentation, and communication skills
Responsibilities
- Conduct move-in, 3-day, semi-annual, and change-of-condition assessments
- Perform monthly wellness visits and monitor health trends
- Follow up on falls, incidents, and hospital returns
- Transcribe physician orders and ensure accurate MAR/TAR documentation
- Complete medication audits and oversee medication compliance
- Partner with families, physicians, and the Resident Care Director
- Support infection control and regulatory standards
- Educate and guide care staff on documentation and medication practices
