Requirements

  • Graduate of an accredited Associate Degree Nursing program
  • Current state LVN/LPN license in good standing
  • Minimum of one (1) year of clinical nursing experience
  • Ability to lift, push, pull, and support up to 50 pounds as needed
  • Valid driver's license, reliable transportation, and current auto insurance
  • Willingness to travel locally (approximately 50%)

Nice To Haves

  • Home Health or Hospice experience preferred
  • Strong clinical assessment and communication skills

Responsibilities

  • Providing skilled nursing care under the direction of an RN and physician
  • Following the established Plan of Care and physician orders during each patient visit
  • Monitoring patient conditions and reporting significant changes to the RN Case Manager and care team
  • Educating patients and caregivers on disease management, medications, and self-care techniques
  • Supporting patient safety through individualized care planning and risk prevention strategies
  • Documenting patient assessments, interventions, and medication updates accurately and timely
  • Collaborating closely with physicians, RNs, and interdisciplinary team members to ensure quality outcomes
  • Identifying opportunities to improve patient care and recommending updates to the Plan of Care
  • Helping reduce avoidable hospitalizations and emergency room visits through proactive patient management
  • Delivering compassionate, respectful, and professional care to patients and families
