Requirements

Graduate of an accredited Associate Degree Nursing program
Current state LVN/LPN license in good standing
Minimum of one (1) year of clinical nursing experience
Ability to lift, push, pull, and support up to 50 pounds as needed
Valid driver's license, reliable transportation, and current auto insurance
Willingness to travel locally (approximately 50%)

Nice To Haves

Home Health or Hospice experience preferred
Strong clinical assessment and communication skills

Responsibilities

Providing skilled nursing care under the direction of an RN and physician
Following the established Plan of Care and physician orders during each patient visit
Monitoring patient conditions and reporting significant changes to the RN Case Manager and care team
Educating patients and caregivers on disease management, medications, and self-care techniques
Supporting patient safety through individualized care planning and risk prevention strategies
Documenting patient assessments, interventions, and medication updates accurately and timely
Collaborating closely with physicians, RNs, and interdisciplinary team members to ensure quality outcomes
Identifying opportunities to improve patient care and recommending updates to the Plan of Care
Helping reduce avoidable hospitalizations and emergency room visits through proactive patient management
Delivering compassionate, respectful, and professional care to patients and families
