About The Position

Lone Star Circle of Care (LSCC) is seeking a compassionate and dedicated Licensed Vocational Nurse (LVN) who is passionate about improving the health of our communities. Requirements

  • Graduate of an accredited vocational nursing program
  • Current and valid Texas Licensed Vocational Nurse (LVN) license
  • Current Basic Life Support (BLS) certification through the American Heart Association or American Red Cross

Responsibilities

  • Obtain, evaluate, and document patient histories
  • Measure and record vital signs
  • Assist licensed medical staff and/or Registered Nurses during patient examinations
  • Draw and collect blood samples and prepare specimens for laboratory analysis as needed
  • Prepare and administer medications according to the Five Rights of Medication Administration
  • Perform routine diagnostic and laboratory procedures
  • Manage referrals, including processing, tracking, and closing referrals upon receipt of consultation reports
  • Receive and document STAT calls from Quest and ARA and communicate results to providers
  • Complete data entry within the Electronic Health Record (EHR) system, including lab results, hemoglobin, and lead screenings
  • Coordinate authorizations and identify in-network service providers for speech, occupational, and physical therapy referrals
  • Perform other duties as assigned
