About The Position

The Licensed Vocational Nurse (LVN) provides direct nursing care to residents and supervises the day-to-day nursing activities performed by Certified Nursing Assistants (CNAs). Care is delivered in accordance with current federal, state, and local regulations, facility policies, and under the direction of the Director of Nursing (DON) or Nurse Supervisor to ensure the highest quality of resident care. Requirements

  • Graduate of an approved LVN/LPN program
  • Current, active, and unencumbered LVN license in the State of California
  • Current CPR certification

  • Long-term care or skilled nursing experience preferred
  • Knowledge of PCC preferred

Responsibilities

  • Provide direct nursing care in accordance with resident assessments and care plans
  • Supervise and support CNAs to ensure quality care delivery
  • Administer medications and treatments as ordered by physicians
  • Accurately document nursing care, medication administration, and resident status
  • Monitor residents for changes in condition and report findings promptly
  • Participate in admissions, transfers, and discharges as assigned
  • Ensure compliance with infection control, safety, and sanitation standards
  • Communicate effectively with residents, families, physicians, and interdisciplinary team members
  • Assist with restorative and rehabilitative nursing programs
  • Respond appropriately to emergencies and resident needs
  • Maintain resident confidentiality and comply with HIPAA regulations
  • Provides direct supervision to CNAs during assigned shift
  • Ensures staff adhere to policies, procedures, and scope of practice
