Requirements

Active California Licensed Vocational Nurse (LVN) license in good standing
Graduate of an accredited vocational nursing program
Minimum 1 year of nursing experience (Home Health experience preferred)
Valid driver’s license, reliable transportation, and auto insurance
Strong communication, time management, and documentation skills
Comfortable using electronic medical records (EMR systems)

Nice To Haves

Home Health experience preferred
Experience with geriatric patients, senior care, or dementia care preferred

Responsibilities

Provide skilled home health nursing care based on individualized care plans
Deliver care to geriatric and senior patients in home-based and assisted living settings
Accurately document visits, treatments, and assessments in the EMR system
Collaborate with physicians, RNs, families, and interdisciplinary care teams
Support patients with chronic conditions, recovery, and dementia care needs
Ensure compliance with California home health regulations and company policies
Promote patient independence, safety, and overall well-being
