About The Position

The Licensed Vocational Nurse (LVN) is an essential part of the team at Home Care Providers of Texas, where nurses are a top priority and quality care is paramount. LVNs are responsible for providing and documenting skilled nursing care in accordance with the developed care plan and physicians' orders, while adhering to company standards and professional boundaries. This PRN position serves the Henderson, TX and surrounding areas, specifically floating between Henderson, Tyler, and Gladewater. The company utilizes the Roger app to streamline documentation, reducing time spent on paperwork by 50% and allowing clinicians to focus more on patient care.

Requirements

Active LVN License (Texas)
Active CPR Certificate
Home Health Experience – Prior experience in geriatric home health care.
Self-Starter with Organizational Skill
Compassionate Care Approach
Valid Drivers License

Responsibilities

Perform timely and accurate skilled nursing visits in accordance with the patient's individualized plan of care.
Assist the RN Case Manager in developing, reviewing, and updating the patient’s plan of care based on clinical findings and patient needs.
Administer nursing interventions and treatments as outlined in the care plan, ensuring consistent, compassionate, and evidence-based care.
Observe and document the patient’s condition and response to treatments, promptly reporting any changes or concerns to the RN Case Manager and/or physician.
Provide clear, ongoing education and support to patients, families, and caregivers regarding the plan of care, treatment procedures, and self-care techniques.
