About The Position

Delta Post Acute and Rehabilitation is hiring a Licensed Vocational Nurse (LVN) for a night schedule. The LVN provides compassionate nursing care, monitors and assesses patients, collaborates with physicians and other nurses on individualized care plans, performs routine procedures, documents patient information, and administers medications and treatments according to physician orders. Responsibilities

• Monitor patients’ conditions and assess their needs to provide appropriate care.
• Provide nursing care in a caring and compassionate manner.
• Collaborate with physicians and other nurses to devise individualized patient care plans.
• Perform routine procedures, including blood pressure measurements and administering injections.
• Fill in patients’ charts.
• Adjust and administer patients’ medication and provide treatments according to physicians’ orders.
