About The Position

The Meadows of Napa Valley, a not-for-profit Life Plan Community, is seeking a Licensed Vocational Nurse to join their team. This role involves providing person-centered, individualized, and clinically competent care to residents. The nurse will work with a support team to ensure resident needs are met in accordance with regulations. The position offers a pay rate of $32.00 - $37.66 / hr DOE.

Requirements

  • Minimum of 1 year’s professional nursing experience, especially in long term care.
  • Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) license from accredited nursing program, school or university.
  • Current CPR certification is required.
  • Demonstrated understanding of functions, organization, policy, regulations and procedures of a long-term care/skilled nursing facility.
  • Competent in use of electronic health records and other clinical software.

Responsibilities

  • Understand and advocate for resident rights.
  • Delegate and assign tasks to team members, ensuring accountability and communication.
  • Provide medication and treatment administration for an assigned group of residents.
  • Manage medication administration by maintaining standards of practice, understanding universal precautions, setting up and passing medications, and observing residents for drug effects.
  • Identify, gather information, and report changes in resident conditions.
  • Document findings, notify physician and responsible party, and create resident-specific Service Plans.
  • Receive, transcribe, and review physician orders for accuracy, including reconciliation of medications.
  • Write and develop Service Plans to identify resident needs, approaches, and goals.
  • Read and follow individualized care and Activities of Daily Living (ADL) plans.
  • Support the admission of new residents, including identifying immediate needs and educating caregivers.
  • Promote resident independence.
  • Enhance and maintain a physical, social, and psychological environment conducive to resident welfare.
  • Review and maintain 24hr report and other nursing documentation.
  • Attend and participate in continuing education programs and in-services.
  • Ensure proper documentation by entering resident information into electronic medical records.
  • Be aware of safety and follow up on identified risks.
  • Respond to emergency calls on campus per protocol.
  • Initiate and complete incident reports.
  • Assist team members with direct care as needed.
  • Supervise, assist, and implement infection control measures.
  • Perform other duties as assigned.
