About The Position

The Licensed Vocational Nurse (LVN) provides direct nursing care to residents under the direction of the Wellness Director and in accordance with physician orders, state regulations, and community policies. The LVN supports resident health, safety, and well-being while collaborating with the interdisciplinary care team.

Requirements

• Current California Licensed Vocational Nurse (LVN) license in good standing.
• Current CPR certification.
• Strong clinical assessment and documentation skills.
• Excellent communication and interpersonal skills.
• Ability to work collaboratively with residents, families, and team members.

Nice To Haves

• Experience in assisted living, memory care, skilled nursing, or related healthcare settings preferred.
• Medication administration experience.
• Knowledge of state regulations for assisted living and memory care communities.
• Strong organizational and time management skills.
• Compassionate, resident-centered approach to care.

Responsibilities

• Provide resident assessments and monitor changes in condition.
• Administer medications and treatments as prescribed.
• Maintain accurate and timely clinical documentation.
• Communicate resident condition changes to physicians, families, and the care team.
• Assist with resident care planning and implementation.
• Provide education and support to residents, families, and caregivers.
• Ensure compliance with company policies, licensing regulations, and nursing standards.
• Participate in staff meetings, training programs, and continuing education activities.
• Support quality care initiatives and resident safety programs.
