About The Position

Aspire Home Healthcare is seeking caring and dedicated Licensed Vocational Nurses (LVNs) to join their growing home health team. For over 30 years, Aspire Home Healthcare has been committed to putting patients first, providing compassionate, around-the-clock care that helps patients maintain their independence in the comfort of their homes. The LVN is responsible for providing direct patient care and case management to an assigned caseload in accordance with physician orders. You’ll help patients heal safely at home by delivering skilled, compassionate care and supporting the care team in developing individualized treatment plans.

Requirements

• Active LPN license in CA
• Physical exam within the past 6 months
• Current TB test (within 12 months) and CPR certification
• Valid driver’s license and proof of auto insurance
• Ability to attend a 2-day orientation at our Yuba City Locations

Responsibilities

• Obtain medical history and assess patient conditions
• Conduct physical assessments including vitals and mental status
• Provide skilled nursing care per physician’s plan of care
• Maintain patient confidentiality in compliance with HIPAA
• Evaluate patient progress and communicate updates to the team
• Submit all visit notes and documentation promptly
• Collaborate with RNs and other care team members
• Educate patients and families on care plans and home management
