About The Position

Aspire Home Healthcare, with over 30 years of experience, is dedicated to patient-centered care, enabling patients to maintain independence at home. They are seeking compassionate and dedicated Licensed Vocational Nurses (LVNs) to join their expanding home health team. The LPN will provide direct patient care and case management for their assigned caseload, following physician's orders. This role involves ensuring patients heal safely at home through skilled, compassionate care and contributing to individualized treatment plans.

Requirements

• Active LPN license in CA
• Physical exam within the past 6 months
• Current TB test (within 12 months) and CPR certification
• Valid driver’s license and proof of auto insurance

Responsibilities

• Obtain medical history and assess patient conditions
• Conduct physical assessments including vitals and mental status
• Provide skilled nursing care per physician’s plan of care
• Maintain patient confidentiality in compliance with HIPAA
• Evaluate patient progress and communicate updates to the team
• Submit all visit notes and documentation promptly
• Collaborate with RNs and other care team members
• Educate patients and families on care plans and home management
