About The Position

Garden City Health Care Center, a skilled nursing facility located in Modesto, CA, is seeking qualified and caring Licensed Vocational Nurses (LVNs) to join their nursing team. The role involves playing a critical part in resident care delivery, team leadership, and compliance with clinical standards in a supportive environment that promotes professional growth. The position is per diem with PM shifts available.

Requirements

Current LVN license in good standing with the California Board of Nursing
CPR certification
Ability to lift, push, and move up to 25 lbs and assist in emergency situations
Must pass background screening and meet facility health requirements

Nice To Haves

Experience in a skilled nursing or long-term care setting
Knowledge of PCC (PointClickCare)

Responsibilities

Supervise and direct Certified Nursing Assistants during assigned shifts
Oversee daily operations and ensure adherence to care plans and facility policies
Participate in staff evaluations, orientation, training, and quality assurance initiatives
Communicate effectively with residents, families, physicians, and interdisciplinary teams
Respond to incidents, complete documentation, and escalate concerns to Nurse Supervisor
Administer medications and treatments per physician orders
Monitor and chart resident health status and care interventions
Support admissions, discharges, and transfers with proper documentation
Participate in care planning and discharge planning processes
Maintain accurate records in compliance with state, federal, and facility requirements
Enforce hand hygiene, infection control, and safety procedures
Participate in emergency preparedness and fire/disaster drills
Maintain a clean, safe, and sanitary work environment
Ensure proper use of PPE and disposal of medical waste
