About The Position

Compassionate Home Health LTD is seeking a motivated and compassionate Per Diem Home Health Licensed Vocational Nurse (LVN) to provide skilled nursing care to patients in their homes throughout the Greater Sacramento Area. The Home Health LVN is responsible for providing skilled nursing services to assigned patients under the direction and supervision of the Registered Nurse Case Manager and in accordance with physician orders, individualized plans of care, and agency policies and procedures. The LVN provides compassionate, patient-centered care including skilled nursing visits, medication management, wound care, patient education, and monitoring of patient conditions. The LVN collaborates with the interdisciplinary team to promote positive patient outcomes and ensure care is delivered in compliance with accrediting organization standards, Federal and State regulations, reimbursement guidelines, and agency requirements. Participates in Quality Assessment and Performance Improvement (QAPI) activities.

Requirements

  • Current Licensed Vocational Nurse (LVN) license in the State of California
  • Current BLS/CPR certification
  • Valid California Driver’s License, current automobile insurance, and reliable transportation
  • Strong clinical assessment and communication skills
  • Ability to work independently while collaborating with the care team
  • Knowledge of current Federal, State, accreditation, and agency requirements related to home healthcare
  • Computer skills and ability to document using electronic medical record (EMR) systems
  • Strong organization, time management, and documentation skills
  • Ability to provide compassionate care in a patient’s home environment

Nice To Haves

  • Prior home health experience preferred but not required
  • Wound care experience preferred; training and support available for LVNs interested in expanding their wound care skills
  • Experience with skilled nursing visits, medication management, patient education, catheter care, and chronic disease management preferred

Responsibilities

  • Providing skilled nursing visits to patients in their homes according to the established plan of care
  • Performing patient assessments and reporting changes in patient condition to the RN Case Manager and physician as appropriate
  • Providing wound assessments, wound care treatments, and monitoring patient progress according to physician orders and established protocols
  • Assisting with the management of patients with complex medical conditions and chronic illnesses
  • Monitoring vital signs, medications, symptoms, and overall patient progress
  • Performing treatments and procedures within the LVN scope of practice
  • Reinforcing patient and caregiver education related to medications, treatments, disease management, safety, and preventative care
  • Collecting and documenting accurate clinical information in the electronic medical record (EMR)
  • Completing timely and accurate visit documentation, including skilled nursing notes and required clinical updates
  • Communicating effectively with patients, families, physicians, RN Case Managers, and the interdisciplinary team
  • Collaborating with the RN Case Manager regarding changes in patient status, care needs, and patient progress
  • Following infection prevention, safety, and quality standards
  • Participating in agency training, education, and Quality Assessment and Performance Improvement (QAPI) activities
