Licensed Vocational Nurse (LVN)
About The Position
STAT Home Health is seeking PRN Licensed Vocational Nurses (LVN) for Pearland, TX. This role involves providing nursing and supportive care in accordance with the Interdisciplinary Plan of Care, under the direction of the RN. The LVN will communicate with physicians and the home health team to coordinate patient care and utilize community resources. Documentation of all services provided is essential, along with knowledge of Medicare/Medicaid regulations for home health benefits. Reporting changes in patient condition to the RN is a key responsibility. The ideal candidate will possess great time management skills.
Requirements
- Current Texas LVN license
- Minimum of 2 years as a Licensed Vocational Nurse (LVN)
- Have dependable transportation, driver’s license, and car insurance
Nice To Haves
- Home Health experience preferred
Responsibilities
- Provides nursing and supportive care in accordance with the Interdisciplinary Plan of Care, under the direction of the RN.
- Communicates with the attending physicians, other home health team members, and other agencies as needed to coordinate optimal care and use of community resources for the patient/family.
- Document all services provided.
- Demonstrates knowledge of Medicare/Medicaid regulations for home health benefits.
- Report changes in patient condition and needs to RN
