Requirements

  • Current Licensed Vocational Nurse license in good standing in the State of California required.
  • One to three years of clinical experience with SMI and/or complex populations preferred.
  • Strong ability to remain professional and effective in stressful or conflict situations, including working with uncooperative clients.
  • Familiarity with Electronic Health Records (EHR) and other technology tools.
  • Understanding of confidentiality standards and HIPAA compliance.
  • Current CPR Certification preferred.
  • Proof of valid California Driver’s License, clean driving record, and insurability under Alcott’s policy required.
  • Live Scan clearance required.

Responsibilities

  • Provide basic nursing services, including injections, medication administration, and monitoring.
  • Coordinate and schedule primary care and specialty medical appointments.
  • Maintain medication logs in collaboration with the Program Manager for clients receiving psychiatry services.
  • Coordinate with pharmacies to ensure timely delivery of client medications.
  • Offer patient teaching and supportive services to increase compliance with prescribed medication regimens.
  • Work as part of an interdisciplinary team to deliver integrated care to clients with medical, mental health, and substance use needs.
  • Communicate effectively with internal and external providers to ensure continuity of care.
  • Provide crisis intervention and triage clients to emergency services when necessary.
  • Participate in weekly staff meetings and individual supervision sessions.
  • Accurately document services in real time in the electronic health record or charting system.
  • Remain knowledgeable about Medi-Cal benefits and assist clients with navigating the healthcare system.
  • Maintain compliance with HIPAA and agency confidentiality standards.
  • Perform additional tasks as assigned by the Director to support program operations.
