Requirements

  • Current and unrestricted California Licensed Vocational Nurse license required.
  • Experience in health sciences, social services, mental health, or a related field.
  • Strong understanding of confidentiality and nondisclosure requirements under HIPAA.
  • Familiarity with Electronic Health Records (EHR) and other technology tools.
  • Ability to perform effectively under circumstances of emotional stress or conflict, including when working with uncooperative clients.
  • Strong written and verbal communication, organizational, documentation, and interpersonal skills.
  • Ability to collaborate effectively with interdisciplinary teams and external healthcare providers.
  • Valid California Driver’s License, clean driving record, and insurability under Alcott’s automobile insurance policy required.
  • Successful Live Scan clearance required.

Nice To Haves

  • One to three (1–3) years of clinical experience working with individuals with serious mental illness and/or other complex needs is preferred.
  • Current CPR certification is preferred.
  • Military-experienced candidates are strongly encouraged to apply.

Responsibilities

  • Provide basic nursing services, including injections, medication administration, and health monitoring.
  • Coordinate and schedule primary care and specialty medical appointments to support continuity of care.
  • Maintain medication logs in collaboration with the Program Manager for clients receiving psychiatry services from The People Concern.
  • Work with pharmacies to ensure the timely delivery of client medications.
  • Provide patient education and supportive services to increase compliance with prescribed medication regimens.
  • Work as part of an interdisciplinary team to provide comprehensive care to clients with co-occurring medical, mental health, and substance use needs.
  • Communicate effectively with internal staff and external healthcare providers to coordinate services and address client needs.
  • Provide crisis intervention and triage clients to emergency services, including police, paramedics, or the fire department, when necessary.
  • Attend weekly staff meetings and individual supervision sessions.
  • Document all services accurately and in real time using the designated electronic health record or charting system.
  • Maintain knowledge of Medi-Cal benefits and assist clients in navigating the healthcare system.
  • Maintain compliance with HIPAA requirements and all agency confidentiality and nondisclosure standards.
  • Perform additional tasks as assigned by the Director to support client care and program operations.
