Requirements
- Current and unrestricted California Licensed Vocational Nurse license required.
- Experience in health sciences, social services, mental health, or a related field.
- Strong understanding of confidentiality and nondisclosure requirements under HIPAA.
- Familiarity with Electronic Health Records (EHR) and other technology tools.
- Ability to perform effectively under circumstances of emotional stress or conflict, including when working with uncooperative clients.
- Strong written and verbal communication, organizational, documentation, and interpersonal skills.
- Ability to collaborate effectively with interdisciplinary teams and external healthcare providers.
- Valid California Driver’s License, clean driving record, and insurability under Alcott’s automobile insurance policy required.
- Successful Live Scan clearance required.
Nice To Haves
- One to three (1–3) years of clinical experience working with individuals with serious mental illness and/or other complex needs is preferred.
- Current CPR certification is preferred.
- Military-experienced candidates are strongly encouraged to apply.
Responsibilities
- Provide basic nursing services, including injections, medication administration, and health monitoring.
- Coordinate and schedule primary care and specialty medical appointments to support continuity of care.
- Maintain medication logs in collaboration with the Program Manager for clients receiving psychiatry services from The People Concern.
- Work with pharmacies to ensure the timely delivery of client medications.
- Provide patient education and supportive services to increase compliance with prescribed medication regimens.
- Work as part of an interdisciplinary team to provide comprehensive care to clients with co-occurring medical, mental health, and substance use needs.
- Communicate effectively with internal staff and external healthcare providers to coordinate services and address client needs.
- Provide crisis intervention and triage clients to emergency services, including police, paramedics, or the fire department, when necessary.
- Attend weekly staff meetings and individual supervision sessions.
- Document all services accurately and in real time using the designated electronic health record or charting system.
- Maintain knowledge of Medi-Cal benefits and assist clients in navigating the healthcare system.
- Maintain compliance with HIPAA requirements and all agency confidentiality and nondisclosure standards.
- Perform additional tasks as assigned by the Director to support client care and program operations.
