Requirements
- Current California LVN license in good standing
- Current CPR/BLS certification
- Skilled nursing, rehabilitation, or post-acute care experience preferred
- Strong clinical judgment and assessment skills
- Excellent communication and interpersonal abilities
- Ability to work effectively in a team-oriented environment
- Commitment to providing outstanding resident-centered care
Responsibilities
- Provide direct nursing care in accordance with physician orders and individualized care plans
- Administer medications, treatments, and nursing procedures safely and accurately
- Monitor residents’ conditions and promptly communicate changes to physicians and nursing leadership
- Complete timely and accurate clinical documentation
- Assist with admissions, transfers, and discharges
- Collaborate with CNAs, therapists, physicians, and other healthcare professionals to coordinate resident care
- Maintain compliance with facility policies, infection control standards, and regulatory requirements
- Provide compassionate support and education to residents and their families
