About The Position

For over 20 years, Lorian Health has been a trusted leader in home health care, dedicated to enhancing patients’ quality of life and supporting their independence. Licensed Practical/Vocational Nurse (LPN/LVN) – Home Health. We’re looking for a skilled and compassionate LPN/LVN to provide in‑home nursing care under the supervision of a Registered Nurse (RN). You’ll support patients with daily care needs, monitor their condition, and help ensure safe, high‑quality home health services.

Requirements

• Current LPN/LVN license in the state of practice.
• Graduate of an accredited practical/vocational nursing program.
• Current CPR/BLS certification.
• Valid driver’s license, reliable insured vehicle.
• Strong communication, clinical judgment, and ability to work independently.
• Ability to pass required physical exams and annual PPD testing.

Nice To Haves

• Minimum 1 year acute care experience preferred.

Responsibilities

• Provide nursing care according to the RN‑developed Plan of Care.
• Monitor and document changes in patient condition; report to RN/physician as needed.
• Assist with activities of daily living to promote patient independence.
• Educate patients and caregivers within LPN/LVN scope of practice.
• Maintain accurate EMR documentation.
• Participate in case conferences and coordinate care with the home health team.
• Follow infection‑control, HIPAA, and company policies.
