About The Position

Mobile Health Team Inc. is seeking an experienced Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN)- Home Health for a 13-week travel assignment in Pittsfield, MA. The Community Health LPN provides and documents nursing care of the sick and injured at home. The LPN intervenes to promote, maintain or restore health to prevent illness and to effect rehabilitation. The LPN collaborates with other healthcare providers, professionals and community representatives to coordinate care at home. The LPN evaluates the individual’s and families’ response to interventions. Duties require independent action and judgment, utilizing time management, communication, planning, delegating, and problem-solving in providing care.

Requirements

One year acute nursing care, hospital based preferred.
Must possess and demonstrate critical thinking skills.
Must maintain emotional stability in emergency settings.
Experience in home health is required.
Certified Home Health Required Including Proficiency with OASIS in highly preferred.
Must have Massachusetts LPN license.
Infusion Therapy Experience is preferred.
Demonstrated knowledge of the principles and practices of nursing.
Knowledge of and ability to apply community health principles and practice.
Demonstrated ability to perform independently as a clinician with minimal supervision or assistance.
Experience with self-scheduling of caseload preferred.
History of flexibility in assuming varied assignments.
Current Driver License (and insurance) [Required].
In order to effectively provide home care, they must have a working knowledge of home care reimbursement and regulatory guidelines.
Traveler candidate must have their own vehicle. This is a firm requirement.
Ability to exercise independent judgement.

Ability to develop and maintain records and communicate clinical data in verbal and written form.
Willing to be flexible regarding work hours.
Ability to drive an automobile in inclement weather.

Must have and maintain current Basic Life Support (BLS) certification during employment.

Responsibilities

Observe patients, charting and reporting changes in patients' conditions, such as adverse reactions to medication or treatment, and taking any necessary action.
Measure and record patients' vital signs, such as height, weight, temperature, blood pressure, pulse, or respiration.
Administer prescribed medications or start intravenous fluids, noting times and amounts on patients' charts.
Provide and document nursing care of the sick and injured at home.
Intervene to promote, maintain or restore health to prevent illness and to effect rehabilitation.
Collaborate with other healthcare providers, professionals and community representatives to coordinate care at home.
Evaluate the individual’s and families’ response to interventions.
Function independently under general guidance of the Director and follows the guidelines and protocols and/or policies and protocols specific to each unit.
Utilize time management, communication, planning, delegating, and problem solving in providing care.
