About The Position
The Licensed Vocational Nurse (LVN) – Hospice will provide direct patient care under the supervision of a Registered Nurse (RN). This role involves following the plan of care, delivering treatments, and collaborating with the interdisciplinary team to achieve positive patient outcomes. Sequoia Hospice is passionate about delivering exceptional, individualized care and aims to provide life-changing service with dignity and compassion, helping patients and families navigate a smooth transition home. The position is part of the Hospice Care team and requires weekend coverage.
Requirements
- Active CA LVN License
- Minimum 1 year of LVN experience
- Current CPR certification
- Valid driver’s license and insured, reliable vehicle
- Ability to work independently and as part of a team
Nice To Haves
- Hospice experience preferred
Responsibilities
- Implement nursing care based on comprehensive assessment and plan of care
- Document patient services accurately and timely
- Educate patients and families on diagnosis and self-care
- Coordinate hospice services and report needs to the care team
- Manage pain and symptoms; evaluate patient response to medications
- Use equipment and supplies effectively
- Participate in quality improvement and professional development
