About The Position

The primary function of this role is to administer skilled nursing care, under the supervision of a registered nurse, for patients in their place of residence. This position will coordinate care with the interdisciplinary team and patient/caregivers, promote patients' potential during their home health episode, and collaborate with the physician, clinical team, and patient in preparation for discharge.

Requirements

Licensed Vocational Nurse, per the state requirements.
1 year of Home Health experience.
Good communication skills, assessment skills, and strong clinical documentation skills.
Ability to read, write, and speak the English language.
Reliable transportation.
Valid and current auto liability insurance.

Responsibilities

Administer skilled nursing care under the supervision of a registered nurse.
Coordinate care with the interdisciplinary team and patient/caregivers.
Promote patients' potential during their home health episode.
Collaborate with physician, clinical team, and patient in preparation for discharge.
Identify the patient’s physical, psychosocial, and environmental needs.
Participate in planning and implementing care in conjunction with the RN, in accordance with the POC.
Provide care utilizing infection control measures that protect both staff and patient (OSHA).
Record treatments administered in medical record.
Record clinical documentation according to accepted regulatory, corporate and professional guidelines.
Participate in weekly case conferences.
Participate in interdisciplinary communication to ensure that patients’ needs and expectation for services provided are understood and to ensure optimization of service delivery and patient progress.
Provide verbal or written patient status updates to supervising RN prior to reassessment, supervisory and/or discharge visits, and as indicated by patient response to treatment.
Participate in in-service training programs.
Assure patient safety.
Comply with state regulations and national standards.
Complete and sync all documentation by 9:00 AM the following day, 7 days per week.
